Pillows for side sleepers: Choosing the right height and filling

Reviewed by the BEFA Editorial Team — Bedding Manufacturer since 1994

Quick Answer: A side sleeper pillow must be higher than a back sleeper pillow — typically 10–15 cm. The reason: The shoulder creates a hollow space between the head and the mattress that the pillow must fill. The correct pillow height is determined by your shoulder width minus how much you sink into the mattress. Feather pillows and 3-chamber pillows offer the best support because they maintain their height throughout the night. Pure down pillows are usually too soft for side sleepers.

You sleep on your side — like about 60% of Germans. Yet you wake up in the morning with neck pain, shoulder aches, or feeling stiff. In most cases, this isn't due to the mattress. It's due to the pillow.

Side sleepers place the highest demands on a pillow. In no other sleeping position is the distance between the head and the mattress so great — and in no other position does an incorrect pillow affect the neck and spine so directly.

As a bedding manufacturer, we have been producing pillows of various heights, firmness levels, and fillings since 1994. From production and the feedback of thousands of customers, we know which pillow construction works for side sleepers — and which doesn't.

Why Side Sleepers Need a Higher Pillow

When lying on your side, your body is not flat on the mattress. Your shoulder protrudes upwards, creating a hollow space between your head and the lying surface. If the pillow does not fill this hollow space, your cervical spine will bend downwards. The result: Your neck will be unilaterally strained throughout the night.

A pillow that is too low causes the head to sink. A pillow that is too high pushes the head upwards. In both cases, the spine is not straight — and you will feel it the next morning.

The Goal: A Straight Spine in the Side Position

When viewed from the side, the spine should form a straight line — from the back of the head, through the neck, to the tailbone. This can only be achieved if the pillow height precisely compensates for the distance between the shoulder and the head. This distance depends on two factors:

  • Your shoulder width: The wider your shoulders, the higher the pillow needs to be.
  • Sinking into the mattress: A soft mattress, into which the shoulder sinks deeply, reduces the hollow space. A firm mattress increases it.

These correlations are confirmed by the Aktion Gesunder Rücken e.V. (AGR), which tests and recommends pillows according to ergonomic criteria.

From our production: In our pillow manufacturing, we see the difference every day. A pillow for side sleepers receives significantly more filling material than the same type of pillow for back sleepers. For our feather pillow in 40x80 cm, this means 750 g of feathers — enough to maintain a stable height of 10–13 cm.

Calculating the Correct Pillow Height

The optimal pillow height for side sleepers can be approximated:

Pillow Height = Shoulder Width − Shoulder Sink into Mattress

In practice, this means:

  • Narrow shoulders (under 40 cm shoulder width) + soft mattress: approx. 8–10 cm pillow height
  • Average shoulders (40–48 cm) + medium-firm mattress: approx. 10–13 cm pillow height
  • Wide shoulders (above 48 cm) + firm mattress: approx. 13–15 cm pillow height

How to Measure Correctly

Lie on your mattress in your normal side position — without a pillow. Have someone measure the distance between your ear and the mattress. This distance is your ideal pillow height. Important: Measure on the mattress you actually sleep on. A different mattress will change the value.

If no one can measure: Stand with one shoulder sideways against a wall. Lay your head straight — the distance between your ear and the wall surface roughly corresponds to your pillow height. Subtract 1–3 cm, depending on how soft your mattress is.

Which Filling for Side Sleepers?

The filling determines how stably a pillow maintains its height in the side position. For side sleepers, this is the most important factor — because a pillow that sinks under the weight of the head loses its supportive effect.

Feather Pillows — Firm Support for Wide Shoulders

A feather pillow with 100% duck feathers, Class 1, offers the highest support power of all natural fillings. The feather quills have a resilient effect: they push against the weight of the head and keep the pillow at height. For side sleepers with wide shoulders or a firm mattress, this is the best choice.

Our feather pillow in 40x80 cm contains 750 g of feathers — enough for a stable height of 10–13 cm. For side sleepers who need even more support, there is the Extra Firm Feather Pillow with 1,500 g in 40x80 cm — double the filling for maximum height.

From our production: Feathers have a decisive advantage over down for side sleepers: they spring back after pressure. While down becomes flat under pressure and slowly springs back, a feather core maintains its height even after hours. This is why we always recommend a feather-based filling for side sleepers.

Down-Feather Blend — The Compromise

Blend pillows with 15–30% down and 70–85% feathers offer a middle ground: softer than a pure feather pillow, but significantly more supportive than a down pillow. For side sleepers with average shoulder width, this can be a good compromise. However, it lacks the clear separation of support and comfort layers that a 3-chamber pillow offers.

Pure Down Pillows — Often Too Soft for Side Sleepers

A down pillow is soft, light, and conforms to the head — but this is exactly the problem for side sleeping. Down yields under the weight of the head. The height the pillow has when you lie down is significantly reduced within minutes. For side sleepers who need 10–15 cm of support height, a down pillow is usually not sufficient — unless you have very narrow shoulders and a soft mattress.

Why 3-Chamber Pillows are Ideal for Side Sleepers

A 3-chamber pillow is the pillow we, as manufacturers, most frequently recommend for side sleepers. The reason lies in its construction:

  1. Upper Outer Chamber: 90% down, 10% feathers — soft lying surface for the head
  2. Inner Chamber (Support Core): 100% duck feathers Class 1 — keeps the height stable
  3. Lower Outer Chamber: 90% down, 10% feathers — cushioning towards the mattress

These three chambers are separated by distinct baffles. Down and feathers cannot mix. The result: The head rests softly on the outside — while the feather core maintains stable height on the inside.

Why This is Crucial for Side Sleepers

A pure feather pillow perfectly maintains its height — but feels firm. A pure down pillow is pleasantly soft — but sinks in. The 3-chamber pillow solves this dilemma: It combines the supportive power of feathers with the comfort of down, without one compromising the other.

For side sleepers, this specifically means:

  • The height remains stable throughout the night — no sinking, no needing to fluff up
  • The ear rests softly on the down layer — no pressure sensation
  • The spine remains straight because the support height does not vary

Filling Weights of Our 3-Chamber Pillow

So you can gauge the dimensions — here are the filling weights of our 3-chamber down pillow by size:

  • 40x80 cm: 600 g feathers (inner chamber) + 100 g down (outer chambers) = 700 g total
  • 80x80 cm: 1,200 g feathers (inner chamber) + 200 g down (outer chambers) = 1,400 g total

The feather core thus accounts for the majority of the weight. It is precisely this core that makes the pillow so effective for side sleepers.

40x80 or 80x80 cm for Side Sleepers?

For side sleepers, we recommend the 40x80 cm size. The reason: With a 40x80 cm pillow, only the head and neck rest on the pillow. The shoulders remain on the mattress — exactly where they belong in the side position.

An 80x80 pillow tempts you to slide your shoulder onto the pillow. In the side position, the shoulder is then pushed upwards, and the spine bends. This is the opposite of what you need as a side sleeper.

The Surface Effect

There is a second advantage to the smaller size: The filling is distributed over half the surface. With the same amount of filling, a 40x80 pillow will therefore be higher than an 80x80 pillow. For side sleepers who need height, this is a relevant advantage.

Manufacturer Tips: Finding the Perfect Side Sleeper Pillow

From 30 years of pillow manufacturing and the feedback of thousands of customers, we have derived five rules for side sleepers:

1. Support before softness

A soft pillow feels comfortable when you lie down. But for side sleepers, what matters is what happens after 10 minutes: Does the pillow maintain its height? Or is your head already 3 cm lower than at the beginning? Choose a pillow with resilience — feathers or a feather core.

2. Consider the mattress

Pillow and mattress are a system. A soft mattress (cold foam, H2) allows the shoulder to sink in deeply — then you need a lower pillow. A firm mattress (pocket spring, H4) allows the shoulder to sink in very little — then you need a higher pillow. If you buy a new mattress, also check your pillow.

3. Fluffing is not enough

Many people fluff their pillow in the morning and believe that this restores its height. With down pillows, this is partially true. However, with a worn-out pillow, fluffing no longer helps — if the filling has lost its loft, you need a new pillow. High-quality feather pillows last 5-8 years with good care. More on proper care in our guide washing feather pillows.

4. Trial sleep on your side

Always test a new pillow in your actual sleeping position. Lie on your side and check: Is your head straight? Do you feel pressure on your shoulder? Does your neck feel relaxed? Give the pillow 3-5 nights — getting used to a new height takes time.

5. When in doubt: 3-chamber

If you are unsure whether you need a firm or soft pillow — choose a 3-chamber pillow. It combines both and works for the widest range of body types and mattress firmnesses. It is the safest choice for side sleepers.

From our production: The most common feedback from customers who have switched from a down pillow to our 3-chamber pillow: "I no longer wake up with neck pain." The difference is not subtle — it is immediately noticeable. The feather core in the middle prevents the head from sinking overnight. That's exactly what side sleepers need.

Frequently Asked Questions

Which pillow is best for side sleepers?

For side sleepers, a 3-chamber pillow in 40x80 cm is best. The feather core maintains a stable height, while the down cover provides comfort. Alternatively, a feather pillow with 100% feathers offers maximum support — especially for people with broad shoulders.

How high should a pillow be for side sleepers?

A side sleeper pillow should be 10–15 cm high. The exact height depends on your shoulder width and the firmness of your mattress. Narrow shoulders plus soft mattress: 8–10 cm. Broad shoulders plus firm mattress: 13–15 cm.

Why do side sleepers need a different pillow than back sleepers?

When lying on your side, the shoulder creates a hollow space between the head and the mattress. This hollow space is significantly larger than when lying on your back. Therefore, side sleepers need a higher, more supportive pillow — typically 10–15 cm instead of 5–8 cm.

Are down pillows suitable for side sleepers?

Pure down pillows are usually too soft for side sleepers. They sink under the weight of the head and lose their supportive height. For side sleepers with narrow shoulders and a soft mattress, a down pillow can work — but a 3-chamber pillow, which combines down comfort with feather support, is better.

What is better for side sleepers: feather pillow or 3-chamber pillow?

Both are well suited. A feather pillow offers maximum support but feels firm. A 3-chamber pillow combines the support of a feather core with the softness of a down cover — it is the more versatile all-rounder. Those who like a firm sleeping sensation choose the feather pillow. Those who want comfort and support choose the 3-chamber pillow.

Which pillow size is better for side sleepers: 40x80 or 80x80 cm?

40x80 cm is the better choice for side sleepers. With this size, only the head and neck rest on the pillow, while the shoulders remain on the mattress. An 80x80 pillow encourages the shoulder to slide onto the pillow — which leads to a bent spine.

How do I properly care for a side sleeper pillow?

Fluff the pillow daily to distribute the filling evenly. Wash it every 3–6 months at 60 °C and tumble dry with tennis balls — this restores its loft. Detailed care instructions can be found in our guide washing feather pillows.

Can the wrong pillow cause neck pain?

Yes. For side sleepers, the pillow is the most common cause of morning neck pain — even more so than the mattress. A pillow that is too low causes the head to bend down, while a pillow that is too high pushes it upwards. In both cases, the cervical spine is subjected to incorrect strain all night long. If the symptoms persist despite a new pillow, consult an orthopedist.
